Job Type: Part-time, flexible hours (Hours will change week to week.) Hours are based on when we have a new client assessment, every 6 month reassessments based on when client signs up, & Home Care Assistants skills check offs. (Approx. 12 hrs. a month.) As an RN for Grace Care, You would fill in when our main nurse is unavailable.

Your Role at Grace Care:

As a vital part of our leadership and care team, you will:

• Conduct initial nursing assessments and develop personalized care plans for new clients (in partnership with the Care Manager)
• Accurately enter client information into AxisCare (we’ll train you if needed!)
• Perform client reassessments every 6 months
• Serve as an on-call consultant for clinical questions or concerns
• Provide skills visits and training for Home Care Aides (HCAs) every 3 months
• Offer CNA training and ongoing education, as needed
• Participate in quarterly staff meetings and leadership team discussions
• Guide, support, and inspire caregiving staff with professionalism and care
• Help ensure our mission of quality, dignity-driven service is upheld every day

What We’re Looking For:

• Active RN license in Oklahoma
• Experience in home care or similar healthcare setting is a big plus
• Strong communication, time-management, and organizational skills
• Tech-comfortable or willing to learn (especially AxisCare)
• Passion for mentoring caregivers and sharing your clinical knowledge
• Flexible, dependable, and aligned with our values of dignity and compassion
