Provides home based clinical and social services to high risk families referred to SAFE CARE. The goal of the services is to reduce the risk of families for abuse and neglect of their children.

EXAMPLES OF WORK PERFORMED:

Provides  the SAFECARE model to high risk families in the natural home environment. Providers will work around the families schedule and provide services, which are sensitive with respect to family culture.

Utilize case management to advocate for and assists families with community resources and accompanying  the families if necessary to obtain needed services. Assists the family with crisis stabilization and arrange for any follow-up services needed after case termination.

Facilitates families in the development of special skills or increased knowledge in the areas of child health, home safety, parent-child bonding, parent-child interaction, behavior management, child development, advocacy, communication and negotiation skills.

Provides motivational interviewing and problem solving in the areas of Substance Abuse, Domestic Violence and Mental Health (other areas as identified by family).

Providers will meet fidelity to the SAFECARE model within nine months of employment. Providers will have ongoing consultants shadow them up to four times per month to ensure fidelity to model.

Maintains clinical records in a timely manner as required by the program, agency, state or federal funding sources and accreditation authorities.  Completes all required SAFECARE forms and collect data for evaluations and research of the program effectiveness. Documents all activities directly or indirectly related  to serving  families.

Providers will conduct themselves in a professional manner while developing and maintain positive relationships with community partners and with consultants.

Participates in trainings/workshops related to the SAFECARE and in expanding knowledge base in domestic violence, substance abuse, and mental health.

Must return phone calls and pages promptly. Must check and respond to emails daily.

Full time employees are required to spend at least 50% of work hours providing direct service to consumers (consumer contact hours). This includes flexibility in scheduling to accommodate consumer and family needs for evening and/or weekend appointments.

Providers will aid in the recruitment and retention of potential families including but not limited to community fairs, community outreach, and speaking events. This can and will include some nights and weekend events.

Performs other duties as required and/or assigned by supervisor and/or executive staff.

Qualifications

POSITION REQUIREMENTS:

Position requires a Bachelors Degree in Social Work or closely related field and two (2) years experience in human services positions.  A Masters Degree in Social Work or a closely related field is preferred.  Position requires education and experience sufficient for designation as a Psychiatric Social Rehabilitation Specialist (PSRS) in accordance with the standards adopted by the Oklahoma Health Care Authority.

Must have flexibility in work schedule, including being on-call. Casual professional dress is required.  Must be able to bend, stoop and move about facilities freely, climb stairs, lift up to 30 pounds and sit for extended periods. Must possess and provide verification of a valid Oklahoma driver’s license. Must maintain and provide verification of current automobile liability insurance including uninsured motorist coverage. Must possess and maintain current CPR, First Aid and CAPE certifications. Must have current (within past year) TB test; or obtain within 30 days of employment with NorthCare**.** Casual professional dress is required.

Position requires some local travel.  Is required to carry an on-call pager, which is provided by NorthCare. For safety purposes and accessibility, NorthCare requires that all providers carry a cell phone, not to be used while driving. NorthCare does not provide staff with cell phones but provides a monthly stipend to help cover the expense. Amount of stipend varies depending upon full-time or part-time status.

Must be COVID 19 vaccinated and follow CDC guidelines regarding COVID vaccination and Boosters

Must have and/or complete the Core Level Co-occurring Training and Trauma Informed Training via NetSmart. ? Must possess skill in the MS Word processing software and intermediate general computer skills. Must possess basic computer skills, including ability to login to the NorthCare website and perform general word processing (i.e. typing, spell check, copying, pasting and saving information).  Must demonstrate good interpersonal skills and the ability to effectively communicate verbally with English-speaking consumers
