Transitional Living Program Case Manager

Youth & Family Services of North Central Oklahoma, Inc.

Youth and Family Services is seeking a compassionate, organized, and mission driven Transitional Living Program Case Manager to support young adults participating in our Runaway and Homeless Youth Transitional Living Program.

This position plays a key role in helping young people build independence, achieve stability, and move toward long-term success. If you are passionate about trauma-informed care and empowering young adults, we would love to hear from you.

What You Will Do

As a Transitional Living Program Case Manager, you will provide comprehensive, youth centered case management services focused on helping participants achieve safe and stable housing, education and employment goals, life skills development, and long-term well being.

Responsibilities include intake and assessment, individualized service planning, goal tracking, life skills coaching, and aftercare support. You will connect participants to community resources, support their progress toward independence, and maintain accurate and timely documentation in required data systems.

This role also includes facilitating life skills groups and trainings, supporting prevention and skill building efforts, and occasionally presenting curriculum to youth and families. You may also assist with emergency shelter coverage, participate in community outreach, and support agency fundraising efforts.

Qualifications

  • Bachelor’s degree in social work, psychology, human services, or a closely related field preferred.
  • Experience working with runaway and homeless youth, justice involved youth, or at risk adolescents preferred.
  • Strong communication skills, including the ability to facilitate groups, provide training, and engage with community partners.
  • Ability to manage multiple responsibilities while working independently and as part of a team.
  • Valid driver’s license required. Must be able to safely operate agency vehicles, including driving in urban and high traffic areas such as Oklahoma City, Tulsa, and other populated communities.
  • Must successfully pass all required background checks and screenings.
  • Residence within 25 miles of 605 W. Oxford Ave., Enid, OK is required to ensure reliable response during on call assignments and emergencies.

Schedule and Availability

This position requires a flexible schedule based on participant needs and program demands. Some evening and weekend hours are required, including participation in community trainings, events, and occasional on call or emergency response coverage.

Physical and Work Requirements

This position involves work in office, residential, and community settings. Staff must be able to walk for extended periods while accompanying participants in the community, including during appointments and other off site activities.

The role requires the ability to operate a company vehicle in daylight, darkness, and various weather conditions. This is a safety sensitive position due to direct responsibility for participant safety and well being.
